How do we bring ROI to our organizations through interoperability?  The main way is to strategically consume external data to close care gaps and improve HEDIS reporting.  This program looks at data in three ways: 

  • Expanding data exchange options
  • Optimizing connections
  • Consuming the data in strategic ways

WHY DOES THIS MATTER?

The consumption of external data is becoming more important strategically for organizations to improve care coordination, meet HEDIS reporting requirements and close Health Maintenance Care gaps. 

This complicated program is not just about getting data into the system but strategically using it across applications and workflows.

This program will include a series of presentations and discussions to explore expanding connections, optimizing exchanges and how to consume external data for an ROI.

WHAT IS THE TIMING/CADENCE?

  • The majority of these sessions follow a quarterly or monthly cadence and there is no end in sight for what we can discuss. 
  • Most have already begun, but it’s not too late to join.

The purpose of this program is to optimize the consumption of external data, enable richer charts through more connections and improve ROI through closing open Health Maintenance Care Gap closures and improving HEDIS reporting.

Optimizing Data Consumption Programs:

Data Exchange Expansion and Governance

Expand and optimize data exchange between and beyond Epic including TEFCA, Carequality, Aura, Cosmos, and State HIEs.

  • CERM Optimization
  • Epic platform (Aura, Cosmos, etc.)
  • non-Epic data exchanges (TEFCA, Care quality, State HIEs)

Optimizing Connections

POP HEALTH QUERIES

Program to enable RUG coordination to support the expansion of population health queries across the regions.

  • Expand organizations that are querying.
  • Expand organizations that enable others to query their data.
  • Monitor and report on results / ROI.

Bi-Monthly huddles

CE SETTING OPTIMIZATION

Identify and standardize specific CE settings to improve data exchange between Epic trading partners.

  • Define additional settings that will be tracked at the RUG level to support an optimized CE data exchange.

Consuming the Data

LAB RESULTS

Improve HEDIS reporting and care gap closure by standardizing and optimizing the consumption of lab data either through advanced lab mapping, profile mapping or abstraction.

  • Set up profile mapping in WI/IL
  • Coordinate advanced lab mapping.
  • Determine high-priority abstraction solutions.

Quarterly Meetings

PROCEDURE / IMAGING

Improve HEDIS reporting, chart depth and care gap closure by standardizing and optimizing the consumption of imaging data either through IEX, HT Imaging, scanning or DICOM imaging.

  • Includes HT Imaging, IEX, Diagnostic Imaging,
  • Error Management,
  • Mapping collaboration,
  • LOINC gaps and
  • Epic development feedback.

Quarterly meetings.

DEATH DATA EXCHANGE

Improve the exchange of death data by reviewing and incorporating new development in at the RUG level to support future connections via TEFCA / other connections with state vital records.

  • Utilize Epic’s new functionality to support Epic to Epic reconciliation of Living Status
  • Death data is exchanged in a standard fashion across WI and IL Epic orgs.

Bi-Monthly Meetings

STANDARDIZATION EFFORTS

Improve interoperability by defining key areas in which having a standard approach supports interoperability goals.

  • HT Notes
  • Patient Questionnaires
  • Newborn naming standards
