SSM Health in Wisconsin is a health care network that encompasses SSM Health facilities in southern Wisconsin. SSM Health in Wisconsin provides health care services at Dean Medical Group, SSM Health St. Mary's Hospital - Madison, SSM Health St. Mary’s Hospital - Janesville and SSM Health St. Clare Hospital - Baraboo and at two nursing homes, SSM Health St. Mary’s Care Center and SSM Health St. Clare Meadows. SSM Health in Wisconsin also enjoys various affiliations with other hospitals including Columbus Community Hospital, Edgerton Hospital, Stoughton Hospital and Upland Hills Health. A key role of our physician and rural hospital partners is providing convenient local care whenever possible. Our relationship with affiliated hospitals gives patients the opportunity to receive care close to home.  We also host several non-affiliate Connect partners on our Epic instance.
