FROEDTERT / MEDICAL COLLEGE OF WI

The Froedtert & the Medical College of Wisconsin health network is a partnership between Froedtert ThedaCare and the Medical College of Wisconsin, supporting a shared commitment to patient care, innovation, medical research and education. Our health network operates eastern Wisconsin's only academic medical center and adult Level I Trauma Center at Froedtert Hospital, Milwaukee, an internationally recognized training and research center engaged in thousands of clinical trials and studies. The Froedtert & MCW health network, which includes 10 hospital locations, more than 3,400 physicians and clinicians, and more than 45 health centers and clinics, draws patients from throughout the Midwest and the nation.

In addition to Froedtert Hospital, the network includes Froedtert Bluemound Rehabilitation Hospital, Froedtert Holy Family Memorial Hospital, Froedtert Menomonee Falls Hospital, Froedtert West Bend Hospital and Froedtert Community Hospital with locations in Mequon, New Berlin, Oak Creek and Pewaukee. The health network represents the collaboration of Wisconsin’s largest multispecialty physician practice with two community-based physician groups. In 2017, an existing affiliation with the former United Hospital System was expanded to include what is now known as Froedtert Pleasant Prairie Hospital and related clinics in the network. In 2024, Froedtert Health and ThedaCare became one organization, making it possible for the health network to enhance access to care for more people in Wisconsin.

HSHS / PREVERA

Hospital Sisters Health System’s (HSHS) mission is to reveal and embody Christ’s healing love for all people through our high quality, Franciscan health care ministry. HSHS provides state-of-the-art health care to our patients and is dedicated to serving all people, especially the most vulnerable, at each of our 15 Local Systems and physician practices in Illinois (Breese, Decatur, Effingham, Greenville, Highland, Litchfield, O’Fallon, Shelbyville and Springfield) and Wisconsin (Chippewa Falls, Eau Claire, Oconto Falls, Sheboygan, and two in Green Bay). MERCY HEALTH

QUAD MED

QuadMed is the country’s first employer-powered health care provider. Our company was founded in 1991 by Harry Quadracci, founder of parent company Quad/Graphics. Harry was determined to solve a persistent problem: skyrocketing health care costs and fragmented, costly and insufficient care for his employees in the community.

SSM HEALTH

SSM Health is a Catholic, not-for-profit health system serving the comprehensive health needs of communities across the Midwest through one of the largest integrated delivery systems in the nation. With care delivery sites in Illinois, Missouri, Oklahoma, and Wisconsin, SSM Health includes 20 hospitals, 63 outpatient care sites, a pharmacy benefit company, an insurance company, two nursing homes, comprehensive home care and hospice services, a technology company and two Accountable Care Organizations.

SSM Health in Wisconsin is a health care network that encompasses SSM Health facilities in southern Wisconsin. SSM Health in Wisconsin provides health care services at Dean Medical Group, SSM Health St. Mary's Hospital - Madison, SSM Health St. Mary’s Hospital - Janesville and SSM Health St. Clare Hospital - Baraboo and at two nursing homes, SSM Health St. Mary’s Care Center and SSM Health St. Clare Meadows. THEDA CARE

For more than 110 years, ThedaCare® has been committed to improving the health and well-being of the communities it serves in Northeast and Central Wisconsin. The organization delivers care to more than 600,000 residents in 17 counties and employs approximately 7,000 health care professionals. ThedaCare has 180 points of care, including eight hospitals. As an organization committed to being a leader in Population Health, team members are dedicated to empowering people to live their unique, best lives. ThedaCare also partners with communities to understand needs, finding solutions together, and encouraging health awareness and action. ThedaCare is the first in Wisconsin to be a Mayo Clinic Care Network Member, giving specialists the ability to consult with Mayo Clinic experts on a patient’s care. ThedaCare is a not-for-profit health system with a level II trauma center, comprehensive cancer treatment, stroke and cardiac programs, as well as primary care.

UNITY POINT HEALTH

Meriter–UnityPoint Health provides comprehensive, coordinated care to patients throughout the South Central Wisconsin through our clinics, hospital and home care services. Employing more than 3,500 people, Meriter offers primary and specialty care, including heart and vascular, orthopedics, women’s health, and the busiest birthing center in Wisconsin. Meriter is a 2013 recipient of the National Research Corporations’ coveted Innovative Best Practice award, Top Rated Adult Doctors award and Top Rated Pediatric Doctors award.
